SFA CTOに対するKnuckle Wire Technique

Subintimal angioplastyは1987年にBoliaらが考案した、長区間の大腿膝窩動脈閉塞病変に対して意図的にワイヤーを偽腔にすすめ再疎通させる技術である。本手法はknuckle wire techniqueを使うことが多いが、Subintimal angioplastyとknuckle wire techniqueは同義語ではない。欧米では、その簡便さや手技時間の短縮、経済的なこともあり一般的な方法である。しかし時にdistal true lumenにうまく再疎通しないことがあり側副血行路のダメージ、病変長の延長、ステント長の延長などの問題点がある。そこで今回我々は上記の長所と短所を補いあった手法としてIVUSを組み合わせたlogical knuckle wire techniqueについて解説したい。
Subintimal angioplasty was originated by Bolia el al. as a technique for recanalization of long femoropopliteal artery occlusive lesion by intentional subintimal dissection. In generally, subintimal angioplasty is ease to make a success by using knuckle wire technique. Advantages of knuckle wire technique are feasible,safe, short procedural time and cost effectiveness. On the other hand, re-entry into the distal true lumen is the most challenging step in this technique. Here we summarize an overview of logical knuckle wire technique which is combination of IVUS guide.
